Home-Visit Service(1)

In-Home Long-Term Care Service (also called At-Home Long-Term Care Service) is available to persons certified as "Support Needed / Long-Term Care Needed" under Long-Term Care Insurance. The service aims to prevent the need for higher levels of care in the future by helping patients as much as possible to maintain or even reduce their current long-term care needs in everyday life.

 

Home Help Service Nanohana

Patients are visited at home by a long-term care-giving specialist such as a certified care worker or home helper who assists them with everyday needs such as bathing, urination/defecation, meals, etc.

Who Can Use Our Services

● Persons certified as "Support Needed / Long-Term Care Needed" under Long-Term Care Insurance

● Persons not eligible under Long-Term Care Insurance but who have difficulty with everyday living due to mental or physical disability.

Services

 ● Bodily Care

 Assistance that entails direct physical contact with persons using the service. Such assistance  includes bathing, urination/defecation, and meals.

 ● Everyday Living Support

 Assistance that entails help with housework, such as cleaning, and other things that need  taking care of in everyday life.

 

 

 

Nanohana Home-Visit Nursing Station

So that people who are sick or have disabilities can spend their time recuperating on their own terms in the familiar comfort of the home and local community they know, this service provides nurses who visit patients where they live to provide nursing care that supports their recuperation and help them return to independent living.

Services

1. Mental and Physical Observation

1) Taking measurements of body temperature, pulse, breathing, blood pressure, etc.

2) Providing intravenous drip as directed by a primary physician

3) Communicating with the primary physician as needed based upon the patient's condition

2. Actual Nursing and Long-Term Care Provided

1) Maintaining bodily hygiene (sponge bath, etc.)

2) Urination (catheter administration)

3) Defecation control and guidance

4) Bed sore prevention and care

5) Rehabilitation

6) Managing use of medical devices

4. 24-Hour On-Call System

  In the event a patient's condition changes, we have a system that allows

  patients to reach us year-round 24-hours a day.

 

 

Home-Visit Rehabilitation

Patients are visited at home by a specialized therapist who assists them with rehabilitation. The therapist helps patients acquire mobility to prevent loss of strength and maintain bodily function and recommends ways to reorganize the home and utilize self-help devices.
